Team Youth Mental Health Hospital in the Home (Y-HiTH) is a service operating since July 2024 in the North West region, based in Devonport. The team provides intensive mental health care within a young person’s home, working with young people aged 15–24 who may be experiencing an acute episode of mental illness. The aim is to support young people to receive treatment in their home and community rather than an inpatient mental health unit, and to facilitate discharge from hospital when appropriate. The Y-HiTH team works with the young person and their family/significant others to develop an individual treatment plan that supports recovery, with multiple home visits each day and additional support via telephone and telehealth. Young people receive comparable treatment to that from an inpatient admission. Role Engage and collaborate with the young person, family, and MDT to plan, implement, and evaluate nursing care that is evidence-based, person‑centred, and trauma‑informed. Act as an active member of the multidisciplinary team, providing biopsychosocial assessments, therapeutic engagement, monitoring and review of mental state and treatment response, and updates to the care plan in consultation with the young person, family, and MDT. Evaluate the effectiveness of nursing strategies and participate in the development and revision of organisational documentation, policies, protocols, and procedures relating to nursing best practice and the model of care for Y‑HiTH. Complete all required clinical documentation, including assessments, risk assessments, transfer of care, clinical correspondence, and data collection within specified timeframes, mainly via electronic consumer record systems. You will need Current knowledge and experience applying nursing principles in acute youth mental health, in line with the Australian Nursing and Midwifery Council competency standards for Registered Nurses. Knowledge of psychosocial and health needs of young people and application to nursing practice, including maintaining therapeutic relationships. Relevant experience in an acute mental health setting and demonstrated knowledge of legislation such as the Mental Health Act 2013. Details of appointment Multiple permanent, full‑time or part‑time shift worker positions, working up to 76 hours per fortnight, commencing as soon as possible; hours subject to negotiation. Salary $80,524 to $109,175 per annum (Pro rata). Salary range follows the Nurses and Midwives (Tasmania State Service) Agreement 2023. Access to the Grade 4 salary range $102,439‑$109,175 subject to progression to Grade 4. Desirable criteria include possession or pursuit of a specialist tertiary graduate or postgraduate mental health/psychiatric nursing qualification, or completion of a hospital‑based training program that resulted in registration as a psychiatric nurse.
