As a Registered Nurse Transport Coordinator, you will play a vital role in supporting the safe, timely, and efficient coordination of inter-hospital patient transfers across CCLHD. Working within the DPFU, you will collaborate with multidisciplinary teams, clinicians, and external healthcare providers to optimise patient flow and ensure continuity of care. This position is an integral part of the District Directorate of Clinical Operations and contributes to maintaining effective patient movement and capacity management across the district. In This Role You Will Utilise the Patient Flow Portal to coordinate patient transfers and transport arrangements to support safe and efficient patient flow across healthcare facilities. Collaborate with nursing, medical, and multidisciplinary teams to prioritise workflows and support positive patient outcomes. Accurately document, collate, and report healthcare information using electronic patient management systems and medical records. Liaise with external providers including Ambulance Service NSW, Patient Transport Services, Fixed Wing services, private hospitals, and residential aged care facilities to facilitate coordinated transfers of care. Provide clinical advice and support regarding patient flow, transfer processes, and service coordination across the district, while maintaining documentation within the Patient Flow Portal to communicate all patient flow related information. Contribute to practice development initiatives and continuous improvement activities to enhance patient and staff experience. About You Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse (Division 1). Demonstrated clinical experience with advanced nursing skills related to patient flow, transfer of care, and care coordination. Strong communication, negotiation, and interpersonal skills with the ability to work collaboratively across multidisciplinary teams. Proven ability to prioritise competing demands, manage workloads effectively, and work autonomously in a fast-paced environment. Experience using patient management systems and electronic medical records to support effective patient flow and coordination, with knowledge of the Patient Flow Portal highly desirable.
