Are you a dedicated Physiotherapist looking to move away from the clinic and into the heart of the community? We are seeking anAccredited Physiotherapist (4–6 years experience) to join our clinical team. This role is pivotal in delivering high-quality restorative care and functional assessments to our clients under theSupport at Home program. You will have the autonomy to manage your own community caseload, focusing on falls prevention, mobility enhancement, and post-hospital rehabilitation, helping our clients live independently and safely in their own homes. What You’ll Do Conduct comprehensive clinical and functional assessments in a home-based setting. Develop and implement evidence-based treatment plans tailored to aged care needs. Collaborate with our Care Partners and clinical team to ensure holistic client support. Provide expert advice on assistive technology and home modifications. Mentor junior staff and contribute to our clinical "knowledge hub." What We’re Looking For Experience: 4–6 years of post-graduate experience (preferably with a focus on gerontology or community health). Accreditation: Full AHPRA registration as a Physiotherapist. First & CPR Certificate. Australian Criminal History Check Skills: Strong clinical reasoning, excellent documentation skills (Excel proficiency is a plus), and a compassionate, client-cantered approach. Requirements: Valid Blue/Yellow Card (or ability to obtain), NDIS Worker Screening, and a reliable vehicle with a valid Driver’s License. Why Join Us?
