About The Role We are looking for Youth Lead Practitioners to support adolescents and young adults aged 9‑25 as they navigate life with a disability, from adolescence through to early adulthood. As a Youth Lead Practitioner, no two days will be the same. If you were here last week, some of the things you might have done include: Lead the interdisciplinary youth team that plans a young person’s NDIS‑funded therapy package, acting as the main contact for adolescent and young‑adult participants and their families Utilise assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working‑life goals Work within the social model, delivering therapy in diverse settings – from homes and schools to playgrounds, workplaces and community centres Hold meaningful, future‑focused conversations with participants and their supports Empower participants and their families to develop habits and routines Build meaningful relationships with your participants and their supports (family, carers, friends) Document and bill for each session through our online participant management system Attend the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs Meet with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or support Qualifications and Experience To work as a Youth Lead Practitioner, you will have a relevant tertiary qualification and registration.
