The successful candidate will have experience with mental health assessments and therapeutic services, including psycho-education and focused psychological interventions / strategies. As a Clinical Care & Coordination Clinician you will play a key role in the care of people with severe mental illness, by delivering therapeutic mental health services and coordinating integrated care. The CCC Clinician will collaborate, consult and liaise with primary healthcare providers, acute health, emergency services, rehabilitation and support services, family, friends, and carers and/or other agencies to support optimal client outcomes.
