About The Team The School Based Youth Health Nurse Service aims to promote positive health outcomes for young people and their families through the provision of a primary health care service centred around the principles of early intervention, assessment and harm minimisation and prevention. The service provides the opportunity for young people, their parents/ carers, and members of the school community to access a health professional in the school setting for matters relating to the health and well‑being of young people. Nurses provide individual health consultations to support the health literacy and help‑seeking behaviours of young people. The service is delivered in partnership with the Department of Education to state secondary school students, with Nurses supporting school communities to adopt a whole of school approach utilising the health promoting school's framework to address contemporary health and social issues facing young people and their families to help young people make a safe and healthy transition into adulthood. About The Role The Transition Registered Nurse (RN) in School Based Youth Health delivers safe compassionate clinical care, working collaboratively with the Healthcare team to achieve best practice client outcomes. The RN engages in reflective, analytical practice through effective assessment, planning, implementation and evaluation of direct person‑centred care. The RN continually improves their skills and knowledge developing from graduate to highly skilled Registered Nurse. The position of Registered Nurse will: Utilise foundation knowledge and skills specific to Youth Health Nursing. Deliver client services for young people, individuals and groups, within the context of their family and community. Provide clinical and problem‑solving skills in the full scope of youth health nursing practice and their families/carers. Practice consistently with nursing professional standards and those of CHQ HHS. Autonomy Practice independently and interdependently. Assume accountability and responsibility for their own actions and delegation of care to Enrolled nurses, Assistants in Nursing and healthcare workers. Demonstrates evidence of increasing autonomy and exercises greater levels of professional judgement in the clinical environment as the employee moves from a beginning to experienced practitioner.
