About the Role You will travel in your own car, delivering mobile nursing services to home and community care clients throughout Melbourne’s metropolitan area. Daily duties include assessments, wound care, clinical reviews, continence and catheter care, palliative and end‑of‑life care, dementia support, NDIS complex care management, client and carer education, immunisations, and more. The role requires depth and breadth of experience, a passionate and caring holistic approach, and the ability to tailor care to each client’s evolving needs. Experience in critical care, emergency nursing, or community nursing is highly regarded. Responsibilities include identifying issues in documentation, problem‑solving on-site, determining the plan of care in collaboration with the team, responding to client needs, managing escalation, and ensuring effective communication across the care continuum. Advanced Clinical Scope in the Home Chronic wound care, including complex and non‑healing wounds Male and female catheter care (IDC and SPC) Continence assessments and management planning Chronic disease monitoring and clinical oversight within the home environment Palliative and end‑of‑life care, supporting clients and families with dignity and compassion Experience & Clinical Excellence Expected This position is designed for highly experienced Division 1 Registered Nurses with no less than 10 years of current clinical experience.
