Opportunity At Location: Hornsby Ku-ring-gai Hospital, Diabetes Education Team. Responsibilities Provide clinical care and education to patients across the diabetes continuum. Deliver structured and individualised diabetes self‑management education. Support patients with insulin initiation, medication management and glucose monitoring. Promote prevention and early intervention strategies for diabetes‑related complications. Collaborate with multidisciplinary teams to coordinate holistic care. Develop and deliver education programmes for patients, families and staff. Participate in quality improvement initiatives, audits and service evaluation. Maintain accurate documentation and contribute to clinical reporting requirements. Support culturally appropriate and patient‑centred care delivery. Qualifications Registered Nurse – Division 1 with the Australian Health Practitioner Regulation Agency (AHPRA). Demonstrated effective interpersonal, verbal and written communication skills. Current clinical knowledge and clinical problem‑solving ability. Commitment to excellence in nursing practice and continuing professional development. Ability to work effectively within a multidisciplinary team. Understanding of the professional, ethical and legal requirements of the Registered Nurse.
