Position Overview We are seeking a proactive and organised Enrolled Nurse to join our team in a hybrid role combining clinical insight with referral coordination and client onboarding. In this role, you will manage incoming hospital referrals, assess client needs, and facilitate a smooth transition from hospital to home care. You will also be responsible for onboarding new clients into our system, ensuring all documentation and compliance requirements are completed accurately. Key Responsibilities : Receive and manage referrals from hospitals and other healthcare providers Review referral information for completeness, urgency, and service suitability Liaise with hospital discharge planners and stakeholders to gather required information Prioritise referrals and escalate clinical concerns to a Registered Nurse (RN) where required Client Onboarding Coordinate the onboarding process for new clients from referral through to service commencement Enter and maintain accurate client information in internal systems and compliance portals Ensure all required documentation, consents, and care details are completed Arrange initial assessments and coordinate with the care team for service delivery Oranising visit times and cordinating with Scheduling team. Clinical Support Apply clinical knowledge to support safe and appropriate client intake Monitor client information and ensure care plans are established and up to date Identify changes or risks and arrange RN review when needed Administrative & Compliance Maintain accurate, audit‑ready client records Ensure all processes align with organisational policies and regulatory requirements Support continuous improvement in referral and onboarding workflows Qualifications Key Selection Criteria Current registration as an Enrolled Nurse (AHPRA) Current Firt aid and CPR NDIS screening Check ( or willing to apply ) WWC Check ( or willing to apply) Full working rights in Australia Preferred Skills Other preffered skills Strong organisational and time management skills Excellent communication and stakeholder engagement abilities Ability to manage multiple referrals and deadlines Confident using systems and learning new software Excellent computer literacy with attention to detail in maintaining digital records Experience in community care or hospital discharge planning (desirable) What We Offer Supportive and collaborative team environment Full training in systems and processes A dynamic hybrid role combining clinical and coordination responsibilities Opportunity to make a meaningful impact in client transitions from hospital to home Access to a learning and training platform to support ongoing professional development and maintain your CPD hours #J-18808-Ljbffr
