Position Description Position Title Clinical Practice Nurse Location Coledale/Quirindi Reports to Practice Manager Award Nurses Award 2020 Role Purpose The Clinical Practice Nurse to assist in the provision of primary health care services for Walhallow Aboriginal Corporation. The Clinical Practice Nurse is expected to be able to work within developed systems, advise of necessary changes to current clinical systems, implement procedures in accordance to best practice guidelines and legislative requirement, or as directed by the Practice Manager. Selection Criteria Experience working within a General Practice environment An understanding of Medicare Items numbers (MBS items) An understanding of chronic disease in general practice and the associated patient management plans (desirable) Willingness to engage and work effectively with Aboriginal communities Ability to assist general practitioners with daily clinics and minor procedures Assist with pre‑screening of patient’s, health education and promotion An understanding of chronic disease management in general practice and the development of GPMPS/TCAs/reviews (desirable) Ability to manage recall and reminder system (give example) An understanding of Aboriginal Health (desirable) Ability to work independently or as part of a team Ability to work unsupervised and maintain all clinical and legislative guidelines with minimal supervision (give example) An understanding of Infection Control and Accreditation Standards in General Practice (desirable) Immunisation certified (or willingness to obtain) Qualifications and Registrations Tertiary qualification in Nursing Registration with AHPRA Valid driver’s license Working with Children Check – must be supplied prior to commencing employment with updates provided during the period of employment NSW Police History Check – must be supplied prior to commencing employment with updates provided during the period of employment Covid Vaccination – must be supplied prior to commencing employment with updates provided during the period of employment First Aid Key Capabilities Aboriginal and Torres Strait Islander Support Capabilities Delivery of primary health care services with the scope of a Clinical Practice Nurse Assist GP’s with their clinic’s Conduct pre-screening of patient’s prior to GP appointment Conduct Aboriginal health checks, GP Management Plans, Team Care Arrangements, kids health assessments (not limited to) Assist with procedures Wound management and dressings Triage Patient education & health promotion Immunisations (both adult and child) Perform ECG, spirometry, audiometry if trained Ear syringing – if trained Women’s Health procedures & assessments – if trained Ensure consumables, equipment and medications are kept stocked and maintained to the manufacturers standards and conditions including a register of stock Maintain the cleanliness of clinical rooms to a high standard in accordance with national standards Maintain the practice register for recalls, reminders and follow up of test results Excellent communication and documentation skills Able to work as part of and contribute to a team environment Able to work independently and within all legislative guidelines Maintain utmost confidentiality Setting up of clinician’s rooms & restocking of equipment Conduct Emergency trolley and medication stores audits & checks Infection control Ensure that the NHMRC Guidelines on Infection Control in Healthcare are implemented and that organisational policies reflect current practice at all times. Ensure compliance with infection control policies and procedures. Provide formal and opportunistic education to all staff on the principles, policies and procedures of infection control. Financial Management To ensure clinical supplies are ordered within allocated budgets (in accordance with practice manager) Maximise Medicare generation through correct claiming of MBS item numbers Cold Chain Management Oversee and maintain cold chain management in accordance with National Vaccine Storage Guidelines Ensure compliance with Cold Chain policies and procedures Monitoring of daily temperatures, weekly tiny tag audits and annual audits Stock level checks, ordering and rotation of vaccines as per National Guidelines and internal policies General Administration Keep up to date and accurate patient health records Keep up to date recall and reminder systems Work with the practice manager and principals to develop and maintain a suite of practice policies and procedures Ensure integrity of patient filing systems and records is maintained and that practitioners are provided with correct and current patient files. Understand the clinical and social issues involved in providing preventative health care services and negotiate with clients with the aim of achieving optimal outcomes Develop and implement strategies for optimising service delivery Identify gaps in service delivery Expand clinical services in accordance with service delivery gaps (MSOAP, Visiting Specialists, Allied Health) Maintain all CPD requirements Professional and Ethical Capabilities Act at all times to protect the right of the clients, including culture, confidentiality, privacy, individual choice and decision-making Practice within own area of competence and qualifications Ensure that all statistical data / record-keeping and other documentation are completed and forwarded on time Coordinate the clinical accreditation process and liaise with outside services in relation to the accreditation of the practice. Participate in appropriate education to maintain and develop professional expertise, skill and knowledge Communicator and Collaborator Capabilities Play a role in organising community health promotion and awareness raising campaigns.
