Key responsibilities Conduct HCP assessments and reviews in participants’ homes Convert client needs into funding opportunities to maximise package utilisation Build strong relationships with participants, support networks and internal teams Deliver high‑quality, person‑centred care Provide comprehensive case management and advocacy Collaborate with allied health, nursing partners and others to coordinate multidisciplinary supports Manage incidents, feedback and complaints in line with ACQS requirements Record and maintain high quality, comprehensive documentation and case notes About you Qualifications and experience: Qualifications in case management (e.g. Diploma in Nursing, Community Services, Human Services, Ageing or Disability) Ideally 3+ years industry experience Understands the Strengthened Aged Care Quality Standards, navigating funding and clinical escalation pathways Ability to anticipate risk, evaluate impacts and make informed, balanced, confident decisions A natural influencer who engages stakeholders and drives alignment and action Highly organised, agile and adept at managing competing priorities Committed to continuous improvement and service excellence Balances care outcomes with financial sustainability We are an equal opportunity employer and welcome applications from people of all backgrounds. #J-18808-Ljbffr
